Vincristine

For an informed discussion with your doctor or pharmacist. This information collection is not a clinically validated interaction checker. Missing data do not establish safety. Do not change or stop prescribed medicines on the basis of this information. The findings apply to the specific conditions studied; laboratory and animal studies cannot be directly extrapolated to humans.

Active substance: Vincristine

Examples of brand names: Oncovin [USA; historical example]

ATC code: L01CA02

Main use (for orientation): Oncology

Documented research summary

Transport inhibition and antitumour/neuropathy effects are predominantly preclinical and partly contradictory. CBD case reports with PCV/chemoradiotherapy do not establish a causal vincristine interaction; the ABCC1 experiment is exclusively a laboratory measurement.

Uncertainties and limitations

Intravenous Hospira vincristine sulphate; not liposomal. ABCC1 cell assay separate from label knowledge about P-gp.

Shared enzymes must not be equated with clinical evidence of an interaction; CBD/THC findings cannot be extrapolated to other cannabinoids.

Separately recorded findings

5 separately recorded findings. Each statement applies to its documented study context.

CBD – laboratory and animal studies

Finding ID: B0222

Type of result: Preclinical evidence

Observation: Submicromolar CBD blocked vincristine-induced neuronal sensitisation in an experimental model.

Mechanism: Neuronal sensitisation

Study type: Neuronal model

Context studied: Slow, broadly acting nociceptor inhibition.

Clinical interpretation: Model finding, not a recommendation for use

Limitations and uncertainties: Do not equate with clinically demonstrated neuropathy prevention.

Original sources for this finding: Physiological profiling of cannabidiol reveals profound inhibition of sensory neurons. [BMED38815194]

THC and CBD – laboratory and animal studies

Finding ID: B0223

Type of result: Preclinical evidence

Observation: THC, not CBD alone, reduced mechanical hypersensitivity; a low-dose CBD/THC combination showed no corresponding effect in the vincristine model.

Mechanism: Neuropathic pain behaviour

Study type: Mouse model

Context studied: Substance- and model-dependent negative finding is important.

Clinical interpretation: Model finding, not a recommendation for use

Limitations and uncertainties: No blanket synergy claim.

Original sources for this finding: Single and combined effects of Δ9-tetrahydrocannabinol and cannabidiol in a mouse model of chemotherapy-induced neuropathic pain. [BMED28548225]

CBD or CBD/THC – laboratory and animal studies

Finding ID: B0224

Type of result: Preclinical evidence

Observation: Cell studies showed additive/synergistic effects; treatment sequence was important in leukaemia cells.

Mechanism: Antitumour combination

Study type: Human and canine cell models

Context studied: Canine CBD/vincristine models; CBD/THC pairs in human leukaemia cells; CHOP component study.

Clinical interpretation: Model finding, not a recommendation for use

Limitations and uncertainties: Not a CBDA combination or clinical efficacy; chemotherapy before cannabinoids was better than the reverse order in the model investigated.

Original sources for this finding: The effect of cannabidiol on canine neoplastic cell proliferation and mitogen-activated protein kinase activation during autophagy and apoptosis. [BMED33247539]; Anticancer effects of phytocannabinoids used with chemotherapy in leukaemia cells can be improved by altering the sequence of their administration. Cannabis extracts – laboratory and animal studies

Finding ID: B0225

Type of result: Preclinical evidence

Observation: Extracts showed antinociception in rats; another investigation found increased hippocampal pro-inflammatory markers.

Mechanism: Neuropathy/inflammation

Study type: Rat models

Context studied: Different endpoints and complex products.

Clinical interpretation: Model finding, not a recommendation for use

Limitations and uncertainties: No general neuroprotective effect of all cannabinoids.

Original sources for this finding: Cannabis sativa L. Extract Alleviates Neuropathic Pain and Modulates CB1 and CB2 Receptor Expression in Rat. [BMED39334832]; Cannabis sativa L. Extract Increases COX-1, COX-2 and TNF-α in the Hippocampus of Rats with Neuropathic Pain. [BMED39795254]

CBD – clinical signals

Finding ID: B0226

Type of result: Clinical observation – not causal

Observation: Two glioma cases reported favourable courses with multiple treatments including CBD.

Mechanism: Supportive treatment with PCV

Study type: Two uncontrolled cases

Context studied: Chemoradiotherapy and procarbazine/lomustine/vincristine; MGMT/IDH context.

Clinical interpretation: Context-specific evidence

Limitations and uncertainties: Not an isolated vincristine pair finding or interaction prediction.

Original sources for this finding: Case Report: Clinical Outcome and Image Response of Two Patients With Secondary High-Grade Glioma Treated With Chemoradiation, PCV, and Cannabidiol. [BMED30713832]
