Lidocaine

For an informed discussion with your doctor or pharmacist. This information collection is not a clinically validated interaction checker. Missing data do not establish safety. Do not change or stop prescribed medicines on the basis of this information. The findings apply to the specific conditions studied; laboratory and animal studies cannot be directly extrapolated to humans.

Active substance: Lidocaine

Examples of brand names: Xylocain [DE]

ATC code: N01BB02 (local anaesthetic); C01BB01 (antiarrhythmic)

Main use (for orientation): Local anaesthesia; antiarrhythmic

Documented research summary

Pilot study in 35 people: successful dental anaesthesia with lidocaine 2% plus epinephrine in 11/18 cannabis users versus 15/17 non-users, p=0.073; onset/duration among those with successful anaesthesia also did not differ significantly. The small sample does not establish equivalence. A 2026 cell experiment shows additive Nav inhibition with CBD only in ND7/23 cells, not DRG neurons, and increased lidocaine cytotoxicity; not evidence of clinical harm. Liposome combinations and THC/CBD aerosol with lidocaine are formulation-development studies, not isolated systemic drug–drug interaction pharmacokinetics.

Uncertainties and limitations

Cannabis-use classification was not randomised; a dental lidocaine/epinephrine combination does not isolate the lidocaine effect. Topical/liposomal/perineural/systemic uses are not interchangeable. No change in systemic lidocaine AUC with CBD established.

Separately recorded findings

3 separately recorded findings. Each statement applies to its documented study context.

Cannabis, chronic use – clinically investigated

Finding ID: B0158

Type of result: No / minor effect under the conditions studied

Observation: Successful anaesthesia in 11/18 users versus 15/17 non-users (61% versus 88%), p=0.073; among those with successful anaesthesia, onset and duration did not differ significantly.

Mechanism: Local anaesthetic efficacy; not pharmacokinetics

Study type: Pilot study

Context studied: n=35; dental infiltration with 1.7 ml lidocaine 2% plus epinephrine 1:100000.

Clinical interpretation: No established difference in efficacy; equivalence not proven

Limitations and uncertainties: Non-randomised cannabis use, small sample and a combination local anaesthetic.

CBD – laboratory and animal studies

Finding ID: B0159

Type of result: Preclinical mechanism

Observation: Additive sodium-channel inhibition in ND7/23 cells, not DRG neurons; increased lidocaine-induced cytotoxicity.

Mechanism: Nav blockade and cell toxicity

Study type: Patch-clamp/cell-toxicity study

Context studied: CBD Nav1.8 IC50 1.4 µM, TTX-sensitive channels 2.9 µM; 2026.

Clinical interpretation: Clinical transferability unclear

Limitations and uncertainties: No human drug–drug interaction; do not infer safe use as an anaesthetic adjunct.

CBD, liposomal/topical – laboratory and animal studies

Finding ID: B0160

Type of result: Formulation-related preclinical finding

Observation: Micelles-in-liposomes increased dermal retention/permeation of both substances compared with gradient liposomes or free control formulations.

Mechanism: Formulation-dependent skin distribution

Study type: Formulation study

Context studied: Local fixed lidocaine/CBD combinations, 2022.

Clinical interpretation: Do not interpret as a systemic metabolic drug–drug interaction

Limitations and uncertainties: No clinical evidence of efficacy or safety; not transferable to usual separate oral CBD/lidocaine use.
