Active substance: Everolimus
Examples of brand names: Certican [DE]; Afinitor [EU/DE]
ATC code: L04AH02
Main use (for orientation): Transplantation; oncology
Documented research summary
CBD–everolimus directly supported by a phase I study: AUC/Cmax approximately 2.5-fold with CBD 12.5 mg/kg twice daily. Additional TSC cases/series show increased trough concentrations; therapeutic drug monitoring rather than blanket dose reduction.
Uncertainties and limitations
Do not extrapolate the pharmacokinetic magnitude to every CBD oil type, dose or transplantation setting. Abstract 31539915 contains a unit typo, 20.4 g/kg; not adopted.
Separately recorded findings
2 separately recorded findings. Each statement applies to its documented study context.
CBD – clinically studied
Finding ID: B0020
Type of result: Positive finding / indication
Observation: Cmax geometric mean ratio 2.50 (90% CI 2.12–2.94), AUC0–last ratio 2.55 (2.23–2.90), AUC0–∞ ratio 2.45 (2.15–2.80); mean half-life 40.0→33.2 hours.
Mechanism: Increased bioavailability; intestinal P-gp suspected
Study type: Open-label phase I pharmacokinetics
Context studied: n=16; CBD 12.5 mg/kg twice daily; everolimus 5 mg.
Clinical interpretation: High
Limitations and uncertainties: P-gp not directly measured in isolation; long-term patient data absent.

CBD – clinical signals
Finding ID: B0080
Type of result: Positive finding / indication
Observation: Median trough-concentration increase +9.8 ng/ml.
Mechanism: Mechanism not established
Study type: Retrospective case series
Context studied: 25 TSC patients in total: 18 receiving everolimus and 7 receiving sirolimus.
Clinical interpretation: Potentially high
Limitations and uncertainties: Uncontrolled retrospective series; n=18 receiving everolimus, not an overall sample independent of the sirolimus series.
