Colchicine

For an informed discussion with your doctor or pharmacist. This information collection is not a clinically validated interaction checker. Missing data do not establish safety. Do not change or stop prescribed medicines on the basis of this information. The findings apply to the specific conditions studied; laboratory and animal studies cannot be directly extrapolated to humans.

Active substance: Colchicine

Examples of brand names: Colchicum-Dispert [DE]

ATC code: M04AC01

Main use (for orientation): Gout; autoinflammatory diseases

Documented research summary

No isolated colchicine pharmacokinetic interaction with CBD/THC demonstrated. A 2026 phase II study (n=27) used pharmaceutical CBD on stable NSAID/colchicine/corticosteroid background therapy; improvement and one serious event leading to discontinuation, but no randomised or colchicine-specific interaction analysis. Two 2019 FMF cannabis cases describe symptomatic improvement after colchicine resistance, but no concentration measurement or reliably isolated colchicine interaction. A 2026 secondary review does not replace original pair data.

Uncertainties and limitations

The pericarditis full text was directly blocked by browser verification; abstract and indexed original excerpts read, with no colchicine-specific pharmacokinetic/safety figures claimed. High toxicity/narrow safety margin establishes relevance, not the existence of a confirmed CBD pair effect.

Separately recorded findings

1 separately recorded finding. Each statement applies to its documented study context.

Pharmaceutically manufactured oral CBD – insufficient evidence

Finding ID: B0161

Type of result: Co-medication context without an isolated interaction

Observation: An open-label pericarditis study with stable concomitant NSAID/colchicine/corticosteroid therapy reported clinical improvement; one serious event led to CBD discontinuation. No colchicine-specific interaction analysis.

Mechanism: Co-medication context, not an isolated pharmacokinetic interaction

Study type: Open-label phase II study

Context studied: n=27; 8 weeks plus an optional 18-week extension with tapering of background therapy.

Clinical interpretation: Informational context; do not interpret as pair safety or synergy

Limitations and uncertainties: No control group/colchicine pharmacokinetics; accessible abstract and original excerpts, not a fully accessible full text.
